*、投标人资格要求
投标人应具备的资格或业绩：
*、作为联合体牵头人应具备的资格要求：
*）质子治疗系统和技术服务的制造商（和/或授权的代理商、服务商）必须是联合体投标的牵头人。 联合体牵头人应具有制造投标产品的能力，或获得授权经销、服务投标产品的资格。质子治疗系统机型具有成功投运的业绩，有充足的运营资金和技术更新能力，有与招标人广泛合作的意愿和能力，有良好的专业声誉（至少提供*份销售合同复印件和质子治疗中心出具的投入运营证明，作为其业绩的证明资料），设备制造商或/和设备制造商指定的服务商能够长期提供良好的售后运维服务，提供至少**年的售后服务承诺（列明客户清单和售后服务机构，并提供相关证明资料），此前售出投运的设备使用期间无重大事故。
*）制造商（和/或授权的代理商、服务商）所投标的质子治疗系统应是世界知名品牌，具有成熟性、先进性特点，具有国际认同的医疗器械认证或FDA或CE等相关机构出具资质文件，具有适用的集成软件，具有可以延续更新的基础和能力，必须承担国外出口管制许可程序和手续。
*）制造商（和/或授权的代理商、服务商）需提供书面承诺，质子治疗系统符合中国食药监局（CFDA）的技术要求审批原则，符合行政许可的相关要求，并作为主体申报CFDA认证，并承诺向CFDA申报的期限。
*）制造商（和/或授权的代理商、服务商）应承担联合体牵头人的责任和义务。
*、联合体投标应具备的资格要求：
*）投标联合体成员必须包含质子治疗系统和技术服务的制造商（和/或授权的代理商、服务商）、国际建筑设计单位和国内建筑设计单位三方。
*）投标联合体的牵头人必须是质子治疗系统和技术服务的制造商（和/或授权的代理商、服务商），国际建筑设计单位、国内建筑设计单位作为投标联合体的成员参与投标。
*）整体规划、建筑方案设计、质子治疗系统区（方案、扩初）设计、室内精装修设计必须由国际建筑设计单位承担，施工图部分必须由国内建筑设计单位承担，其它设计内容由国际、国内设计院协商负责（须在投标时说明各自负责设计内容），质子治疗系统和技术服务供应商必须对国际建筑设计单位和国内建筑设计单位出具的设计文件进行审查并出具书面同意的确认书。
*）联合体各方必须签订共同投标协议，明确约定各方拟承担的工作和责任，并将共同投标协议连同投标文件一并提交招标人。联合体中标的，联合体各方应当共同与招标人签订项目总合同，在总合同中明确牵头人及联合体各方对中标项目承担连带责任或其他对应责任。招标人可以与联合体成员分别签署与总合同不可分割的分项合同。
*）联合体投标必须提交联合体各方的资格证明文件、共同投标协议并注明牵头人及各方拟承担的工作和责任，并承诺一旦中标，联合体各方将向招标人承担连带责任。
*）联合体各方在同一招标项目包中以自己名义单独投标或者参加其他联合体投标的，相关投标均无效。
*、国际建筑设计单位、国内建筑设计单位应具备的资格要求：
*）国际建筑设计单位必须具有已建成投运的质子治疗中心设计业绩和经验（提供合同复印件和质子中心出具的投入运营证明作为业绩证明资料）。
*）国内建筑设计单位必须具有建筑工程设计甲级资质或工程设计综合甲级资质，参与过质子（或重离子）治疗系统医疗建筑设计项目（该项目施工图已完成），或者参与过三甲医院（须配有直线加速器）建筑设计项目（已建成投用）（提供合同复印件及相关证明材料）。
*）需有使用Autodesk公司的Revit软件进行建筑设计的业绩案例（至少提供一个使用Autodesk公司的Revit软件进行建筑设计的案例的书面证明材料及部分设计成果图纸）（国际建筑设计单位、国内建筑设计单位须分别提供业绩案例）

*.Qualification Requirements For Bidder
Qualifications or Performance：
*.The led of joint venture should have the qualification requirements:
*)The manufacturer (and/or authorized agents, service providers)of proton therapy system and the technical service must be a led of joint venture. The led of joint venture should have the ability to create products for bid , or the authorized qualification of distribution and services of bidding products. Proton therapy system model has successfully been put into operation performance and the company is sufficient in operation capital and capable of technology updating, has the broad cooperation willingness and ability with the tenderer and good professional reputation (provide at least one copy of the sales contract and the operating certificate issued by the proton therapy center, as the proof of its performance data.).The equipment manufacturers and/or equipment manufacturers designated service provider can provide good after-sales maintenance services, at least ** years after-sales service commitment shall be provided (listed customer list and after-sales service agencies, and provide relevant certificates materials), The sold operation equipments shall have no serious accident. *) The proton therapy system manufacturer (and/or authorized agents, service providers) bid should be a world famous brand, mature, advanced, and with international identity certification or FDA or CE qualification documents issued by the relevant institutions, with applicable integrated software, with the basis and capability of contiuity and updating, and must undertake foreign licensing procedures and formalities of export control.
*)The manufacturer (and/or authorized agents, service providers) must provide written commitment that the proton therapy system conform to the technical requirements approval principles of the China Food And Drug Administration (CFDA), and conform to the relevant requirements of the administrative licensing, and declare the CFDA certification as a main body, and promise the declaring timetable .
*)The manufacturer (and/or authorized agents, service providers) must bear the responsibility and obligation of a led of joint venture .
*. A joint venture bid should have the qualifications: *) The bidding joint venture members must include three sides: the manufacturer (and/or authorized agents, service providers)of the proton therapy system , the international architectural design units and the domestic architectural design units.
*) The led of joint venture must be the manufacturer (and/or authorized agents, service providers)of the proton therapy system, the international architectural design unit and the domestic architectural design units who should be as the member of the joint venture to participate in the bid.
*) The international architectural designer shall be responsible for the overall planning, architectural design, the design of proton therapy system area (concept and expansion of the preliminary design) and the design of interior refined decoration. The domestic architectural designer shall be responsible for construction drawings. The international and domestic design units shall be responsible for the other design contents jointly by consultation (the content of their own design must be noted in the bid document) The supplier of the proton therapy system and technical service must review the design documents issued by international and domestic architectural designers and issue a written consent of the confirmation.
*) Parties to a joint venture must sign the agreement for joint bidding, clear and definite the work and responsibility of the parties, and submit the joint bidding agreement together with the bid documents to the tenderer. If a joint venture wins the bid, all parties of the Joint Venture shall sign the contract jointly and stating the leading party, work scope and responsibility of each member, as well as the promise to undertake joint liability or other correspondence liability to the Tenderer for the project .The tenderer can sign integral sub-contracts with members of the joint venture.
*) A joint venture bid must submit the parties’ qualification certification documents, joint bidding agreement, and indicate the led to the joint venture, and the parties’ the work and responsibility, and promise if win the bid, the parties will undertake joint and several liability to the tenderer.
*) If the parties of the joint venture would be single bid in the same project package in one's own name or attend the other joint venture bidding, the bid is invalid.
*.International architectural design unit and the domestic architectural design unit should have the qualifications:
*) The international architectural design units must have the performance and experience of the proton therapy center which has been built and put into operation (provide copy of the contract and proof performance of operational data issued by proton center. *) The domestic architectural design unit must have a First Rate qualification of Construction Engineering Design or Class A qualification of Comprehensive Engineering Design, has participated in the medical architecture design project of proton (or heavy ion) treatment system (the construction drawing of that project has been completed), or has been involved in architecture design of Class * Grade A hospital (which must be equipped with linear accelerator) that has been built and put into use, (provide the copies of contract and the related proof materials)
*) The bidder should have the performance case using Revit software of Autodesk company for architectural design (at least provide one case of written evidence and a part of the design results drawings that use Revit software of Autodesk company for architectural design ). (The international architectural design unit and the domestic building design unit must provide performance case respectively).
